Contact your ISP. They should be able to give you the 3 things you need.
1: machine name or ip address
2: login
3: password
Optional 4th piece, the database you are allowed to use. (Not necessary if you are allowed to see all the databases.

Thanks for the response. I have a quick question, is it possible to get that info from the ISP control panel? When I Log on and go to mySQL I see some connection string information there. I tried the string there but still could not connect.
